•Edema
Edema = chemosis와 같은 말이다.
Congestion은? impaired venous drainnage
Edema는 congestion에 의해 생길수도 있고 / increased permeability에 의해 생길수도 있다. 기전이 2가지인 것이다.

Chemosis는 반드시 venous congestion이 있어야 하는 것은 아니다.
Acute allergic state의 경우 venous congestion없이 chemosis만 일어날 수 있다.
위 fig 3.11 설명을 좀더 정확하게 이야기하면 그림 설명 첫줄에서 congestion - > Venous drainage impairment or Increased permeability라고 했지만, 엄밀히 말하면 congestion은 Venous drainage impairment가 원인일때를 가리킨다고 봐야 할 것 같다. 그래야 다른 책들과도 설명이 모순되지 않는다.
Hyperemia
Hyperemia: 급성염증 또는 만성 자극으로 인해 혈관의 calibre, tortuosity가 증가한 상태.
Bright red color로 보인다.
**종종 vascular permeability와 edema, 또는 celluar infiltration과 동반된다.
-> vascular permeability가 edema를 일으켰지.
그래서 fig 3.9 마지막줄에서 vascular permeability and edema라고 한 것은 같은 말을 써 놓은 것 같다
(그림 옆에는 같은게 아니다 라고 내가 노트해놓았는데, 여기서는 같은 뜻으로 쓰인 것 같다)
infiltration 없이 Hyperemia만 있는 경우는 다음의 상황이다.
->Hyperemia without cellular infiltration suggests irritation from physical causes, such as wind, sun, smoke

그러면 Slit lamp상에서 cellular infiltration이 있나 없나가 중요할 거 같은데.
병리책도 봐야하나?
Congestion에 대한 설명은 잘 없는데 한군데 찾음
ㄴ> 위의 책은 congestion을 hyperemia의 좀 심한 단계처럼 사용하고 있음.
그런데 문헌을 잘 찾아봐도 여기말고는 그렇게 쓴 경우가 없는 것을 보면 틀린 거 같음.
Hyperemia 역시 (artery쪽의) dilation이 일어난다. Dilation이 심해진 상태를 표현하기 위해 congestion을 쓴 것 같다.
하지만 congestion은 vein과 관련해서만 써야 할 것 같다.

acute inflammatory infiltrate, that is, neutrophils, eosinophils, and mast cells (Fig. 11), whereas in chronic conjunctivitis the inflammatory response is composed mostly of chronic inflammatory cells, namely lymphocytes and plasma cells

With cellular infiltrate인지 without cellular infiltrate인지 구분을 못하니까.
->대신 discharge, follicle, papillae 같은 것들을 cell infiltration의 증거로 봐야할듯,
•chemosis
Increased systemic vascular permeability, e.g. allergic conditions,
->다시 반복하면, chemosis는 venous congestion / 또는 / increased permeability에 의해서 발생할 수 있다.
그중에서 여기 이문단에서는 Increased permeability만 소개한 것이다.

결론!!
Congestion = impaired venous drainage을 의미한다.
ㄴ>Artery에는 사용하지 않는게 맞을 것 같다
Chemosis(Edema)
ㄴ>1) Increased permeability에 의해서만 일어날 수도 있고,
ㄴ>2) imparied venous drainage(congestion)이 있을 수도 있다.
Increased permeability이 만 발생하면 (white한) chemosis로 보이고.
Congestion이 동반되면 (dark red한) chemosis로 보인다.
Hyperemia: vessel의 number, diameter, tortuosity가 증가한다
(=Injection)
-Increased permeability가 증가하여 edema(chemosis)가 동반될수도 있고
-Cellular infiltrate가 증가하면 discharge로 나타나는 것 같다.
문제는 아래의 교과서
좀 옛날 병리학 책인데, hyperemia도 종류를 구분해놨고, 또 hyperemia를 congestion으로 설명해놓음.
그런데 검색해보니 다른 자료들에서도 passive hyperemia, active hyperemia로 구분해서 설명한 경우가 있음.
Passive hypeemia = congestion(Venous)
active hypermia = 다른 책에 나오는 흔히 사용되는 hypermia (arterial)로 해석하면 이것도 충돌되지 않을듯.
